SCHOOL FORMS
WHEN DOES MY CHILD NEED A SCHOOL FORM TO BE FILLED OUT?
School forms are usually required for any child new to a district or starting kindergarten, entering 6th grade, entering 12th grade, playing school sports, and entering college.
WHERE DO I GET THE FORMS FROM?
Please visit your school’s website for the appropriate forms.
WHO FILLS OUT THE FORMS?
Parents will need to fill out all demographic information for each page of their child’s form and also complete the parent questionnaire. Once this is received then one of our doctors will review your child’s chart and fill out the form.  It typically will take 2-3 days to complete.
WHAT DO I DO IF MY CHILD’S WELLNESS EXAM/PHYSICAL HAS EXPIRED, OR IT HAS NOT YET BEEN 1 YEAR FROM THE PRIOR VISIT BUT THEY NEED SCHOOL FORMS FILLED OUT ASAP?
If your child has had a wellness exam within 365 days we can fill out the form based on the most recent well visit. If your child’s wellness visit is more than a year before the request for forms and we are not able to offer an immediate wellness visit then we can schedule a visit we call the SPORTS PHYSICAL. This is an expedited exam that we will use for the purpose of completing your child’s school forms. This is not the same as the routine wellness visit which is an extensive visit covering all aspects of your child’s health and well-being. Even if a sports physical is done we encourage parents to schedule a routine wellness visit.
