NEW PATIENT PACKET

If you are a new patient please fill out the new patient packet. Our forms are PDF-fillable and do not need to be printed. Simply fill out, save, and email it to us at office@plainsboropeds.com

INSURANCE

Check coverage information with your insurance carrier.  It is the parent’s responsibility to confirm coverage for office visits, vaccines, and other common procedures (Vision screens, hearing screens). Please be prepared to present your insurance card at each visit as we need to visually confirm this information each time your child is seen.  Photos of the front/back of your insurance card will also be accepted.

PRE-VISIT QUESTIONS

Check your email for pre-visit questions.  We are asking that the COVID screening questions be filled out prior to each visit.  For each visit, our staff will send you the appropriate pre-visit questionnaire.  Please be sure to include all of your questions and concerns, and topics you’d like to discuss during your child’s visit.  The more information our doctors have ahead of your visit, the better equipped they will be to care for your child. Not only does this help ensure that our schedule is flowing smoothly but also valuable patient face-to-face time is lost if that time is instead spent in asking and answering routine questions at the time of the visit. We greatly appreciate your cooperation in this matter.

REGISTRATION FORM

Existing patients may also be requested to fill out a new registration form periodically. Our office will let you know what we need to keep your child’s chart up to date.
